With approximately 1 in 3 people being diagnosed with cancer in the Western World more and more people are asking: "why after over 30 years of 'research' are we no nearer to a cure?" So why then 30 years on are doctors not advocating natural cancer cures to their patients? Cancer research charities and pharmaceutical companies are hugely profitable and doctors may either not be knowledgeable about alternative treatments or forbidden from recommending them by their medical board. Because these organisations are so large they have the money and power to dismiss natural cures as being hocus pocus. After all there is no profit for them if they tell us to eat healthily and take more exercise! They are very influential in the media and with governments but we need to start taking responsibility for ourselves and stop trusting them.

Fortunately with the advent of the internet people are now able to do their own research and find a wealth of information about natural cancer treatment. While a lot of people will still dismiss this because of their pre-conditioning many more are starting to wake up to the truth. It makes sense when you think about it that eating a healthy diet, taking more exercise, avoiding processed foods and toxic personal care products will do more to prevent and cure cancer than pumping your body with a load of radiation and poisonous chemicals!
Sometimes all the information that is out there can seem overwhelming to start with but if you start making small changes to your lifestyle then the effects will be cumulative.

There is probably not one method, supplement or food that is going to be an effective natural cancer treatment but a combination of fthese actors. These could be things like quitting smoking, cutting out sugar, eating more raw food and drinking green tea to name but a few. Signs and Symptoms of Bladder Cancer

By now you likely know that cancer, or malignant neoplasm, can affect any part of the body. At this point in time there are so many options available, that people can take advantage of. Natural Cancer Treatment - Avoid Cancer by What We Eat

There is much passing reference to the need to eat more vegetables in order to avoid cancer, but beyond the urging of some health promotion staff little seems to be written in the lay press in any detail at all. In addition to this, or perhaps because of the simplistic message that is out there, many doctors do not yet recognize the importance of diet in relation both to reducing the chances of the disease occurring but also in the care of patients who have already got cancer. They do not recognize that good nutrition is a natural cancer treatment.
Many doctors are wedded to the fast effects of antibiotics. They really, really want any and every drug to be as effective as antibiotics were when they were first used. And if foods cannot give that sort of effect with the majority of people then a goodly number of them will dismiss the impact of food as being "not really important".

However there are many drugs which are not very effective. For example chemotherapy increases survival overall at the 5-year stage by only just over 2%, with it being ineffective in many cancers, despite its ongoing use. With this in mind, any "natural" activity without negative side effects should be embraced with open arms.
There are many foods which have been shown in epidemiological studies to reduce the chances of getting cancer by 25%, 50%, 75% and sometimes even more when you compare high consuming groups with low consuming groups.

There are foods which seem to prevent cells from mutating into cancer cells. There are foods which encourage mutated cells to die off. There are foods which stop the cancer cells from growing, or reduce the speed of growth and foods which stop the cancer cells from moving around the body. In biomedical words foods can be antioxidant, anti-inflammatory, anti-carcinogenic, anti-mutagenic, encouraging apoptosis, cytotoxic, with antimetastatic effects. Of course not all fruit and vegetables have all these effects, but they have enough to be useful.

So which are the most important vegetables and fruits? I would urge you to go for variety and not get hung up on just one or two thinking that will be enough. Plants act synergistically. That is, they work better when you eat a number of them together.

Having said that, the onion family are reported to have some of the strongest effects in preventing cancer. As I trawled through the medical literature there was a huge interest in tomatoes and the cruciferous vegetables, but when I compared the numbers in the studies it seemed that onions, garlic and others in the allium family (like leeks and chives) had the greatest effectiveness.

Tomatoes are good, and so are the cruciferous vegetables - cabbage, red cabbage, cauliflower, broccoli, arugula, beet greens, bok choy, Brussels sprouts, collard greens, garden cress, horseradish, kale, kohlrabi, radishes, rutabaga, Swiss chard and turnips.

Lettuce, carrots, summer and winter squash, oranges, lemons, blueberries, dark raspberries, grapes, cucurmin (of curry fame) and herbs (such as oregano, rosemary and thyme) have all been shown to improve immune functioning and are associated with reduced numbers of cases of cancer in epidemiological studies.

So what does this mean in practice? Ideally fresh fruit and vegetables should be part of every meal. If you can't make it three meals a day, then settle for fruit and vegetables taking up three quarters of your plate at two meals each day.

We are talking about eating - this is not medicine, except that these foods do you good. It is important to choose foods that you enjoy. I really do not like curries or pepper. I do not do well on Indian food, so I do not eat it very often. I was brought up on North European foods and really enjoy those fruit and vegetables my family has eaten for generations. So start by eating fruits and vegetables you enjoy, then go for a little variety and try some that are new to you.

If you are wedded to fried chips, packet foods and hamburgers it might take a little getting used to have three quarters of your plate with fresh vegetables (cooked and raw) with fruit for dessert, but give it a go. It might take a few weeks for your body to develop the enzymes needed to digest your new diet, but once that has happened your body will love you for this change. And as a natural cancer treatment it is both enjoyable and of no more cost than other types of eating.

Cancer Caused of Too Much Acidity in the Body

Biochemist Otto Heinrich Warburg, one of the twentieth century's leading cell biologists, discovered that the root cause of cancer is too much acidity in the body, meaning that the pH, potential hydrogen, in the body is below the normal level of 7.365, which constitutes an "acidic" state. Warburg investigated the metabolism of tumors and the respiration of cells and discovered that cancer cells maintain and thrive in a lower pH, as low as 6.0, due to lactic acid production and elevated CO2. He firmly believed that there was a direct relationship between pH and oxygen. Higher pH, which is Alkaline, means higher concentration of oxygen molecules, while lower pH, which is acidic, means lower concentrations of oxygen...the same oxygen that is needed to maintain healthy cells.

In 1931 he was awarded the Nobel Prize in Medicine for this important discovery. Dr. Warburg was director of the Kaiser Wilhelm Institute (now Max Planck Institute) for cell physiology at Berlin. He investigated the metabolism of tumors and the respiration of cells, particularly cancer cells. "Cancerous tissues are acidic, whereas healthy tissues are alkaline. Water splits into H+ and OH- ions, if there is an excess of H+, it is acidic; if there is an excess of OH- ions, then it is alkaline."

In his work The Metabolism of Tumours Warburg demonstrated that all forms of cancer are characterized by two basic conditions: acidosis and hypoxia (lack of oxygen). "Lack of oxygen and acidosis are two sides of the same coin: where you have one, you have the other."

"All normal cells have an absolute requirement for oxygen, but cancer cells can live without oxygen - a rule without exception."

"Deprive a cell 35% of its oxygen for 48 hours and it may become cancerous." Dr. Warburg has made it clear that the root cause of cancer is oxygen deficiency, which creates an acidic state in the human body. Dr. Warburg also discovered that cancer cells are anaerobic (do not breathe oxygen) and cannot survive in the presence of high levels of oxygen, as found in an alkaline state.

Cancer Revisited, Finding Peace in the "No's" of Our Lives

I asked God to grant me patience. God said No.

The above piece spoke to me in such a way that I related it to a very personal experience. I wanted to share the following story with anyone who is interested. Maybe we can learn to view our lives with a little more humility, hope and courage to meet the challenges we may encounter, be that broken relationships, death or difficult decisions we struggle with daily.

When my granddaughter was diagnosed with lymphoblastic cancer at 5 years of age, our family, church fellowship and friends went into an automatic prayer routine. She endured chemotherapy treatments and the constant high-powered drugs and their awful effects. She rallied and went into remission. There were almost 4 years of relief, but it returned. The chemo, the nausea, the loss of hair, a daily regimen of medicine and treatments, even radiation, resumed. All the while she managed to be her sweet, joyful, upbeat little self. She played like other children except mostly alone, until the danger of germs from other children was at a manageable condition.

She was home-schooled but allowed to have one friend in her "class" occasionally, just so she would not feel completely shut away from her social life. Another rally and renewed hope. Some semblance of normalcy had returned for almost a year and one half. This relentless intruder has reared its ugly head one more time. It appears to have metastasized. The latest tests show her blood counts in dire straits. The doctors, have had to resort to blood transfusions and say the only alternative they have to offer is that same sickening routine and no guarantees of extended life.

Her parents have courageously chosen not to put her through that again. They want the life she has left to be as quality as possible even though it may be short. Michelle will be 11 in the spring, if she makes it. If one did not know she has cancer they would not suspect a thing as long as she has her wig on. She acts like a normal child doing the normal things children do. She spends a good part of her day making gifts for friends and family with her large arts and crafts collection. She has a collection of Fairies and supposedly has "fairy friends." I suspect she's had visions of angels, who surrounded her. She giggles with her playmates and expects she will be a mother someday, like the rest of her friends. She has a "boyfriend," lots of girlfriends and is loved by all who meet her.

The Love in this little girl is the example God has tried to teach mankind, since the beginning of time. There is a kindness, hope, trust, plans, and sweet oblivion to the possible reality. In her short life she has been a friend, a teacher, a consoler, a daughter, a sister, a granddaughter and an example of courage that could put some of us adults to shame. She has endured with patience, created her own happiness and suffered pain many will not know. Her spirit has grown and she has learned to enjoy life in spite of all of the "No's" thrown her way. God knows her storm has been big, but not because she has complained. Like Jesus, at His time of departure from this life, she only knows LOVE.

I don't know about you, but I am ashamed to complain about my little "No's" in life, all because of the example of a loveable, little girl with cancer who has found peace with the "No's" in her life.

Michelle passed away before she reached her 12th birthday. There is a large gaping hole in the lives of those who loved her. Some may think I am reaching with the following, but 2 years after her death, I was strolling through the commissary where I buy my groceries. In the corner of my eye I noticed a beautiful little girl about Michelle's age, as I remember her. The child was alone just staring at me, following me with her eyes. I looked at her and smiled. She smiled back, and then I didn't see her again.

All You Want to Know About Breast Cancer

The particular part of the breast where a cancer originates categorizes that type of breast cancer

*Ductal Carcinoma: The beginnings of this type of cancer is in the cells which line the ducts of the breast, the ducts that supply milk to the nipple; just underneath the nipple and the areola. If the cancer is in situ, that is, it is contained and not invasive out it is called Ductal Carcinoma IN Situ (DCIS). Treatment of such a type of cancer is largely successful and can be typically removed by a lumpectomy. In case the margins of the tumor are devoid of cancer, radiation might be a form of follow-up treatment. If it has become invasive - spread into nearby tissue a mastectomy may be required coupled with chemotherapy.

*Lobular Carcinoma: This variety of breast cancer, as it is named, starts at the lobes (glands) that are located under the ducts far more deeper inside the breasts. These lobes produce milk. If the cancer has not spread and is contained, then it is called Lobular Carcinoma In Situ (LCIS). Depending on whether the cancer is contained or has spread, either lumpectomy perhaps followed by radiation or mastectomy followed by chemotherapy may be the line of treatment.

The next common type of breast cancer are those that have invaded adjacent tissue. They include:

*Invasive (Infiltrating) Cancer of the Breast : These are the cancers that have the potential to spread out from the site of the original tumor and infiltrate into the whole of the breast or even into other parts of the body. Invasive breast cancer come in several different types and subtypes.

*Inflammatory Breast Cancer: A very rare kind of breast cancer but a very aggressive kind. Instead of the typical form of other breast cancers that appear in the form of lumps, this variety takes the form or sheets or nests. It can appear under the skin or in the skin. Unlike the above mentioned two types of cancers, this kind is first treated with chemotherapy followed by surgery. If the cancer id detected early, then the prognosis is good and the rate of survival is gradually increasing.

*Paget's Disease: This type of breast cancer was discovered by Sir James Paget and is so named after him. He was the one who noticed the relationship between the physiological changes of the nipple and the underlying cancer. Typically, the cancer has the appearance of a skin rash or an eczema on the nipple or there is a hardening of the nipple. There may be accompanying itchiness. This itchiness and the dislodging of scabs when scratched are the symptoms of the breast cancer festering under the skin.

Celebrities With Breast Cancer

While no one wants to hear of other people being stricken with cancer, celebrities with breast cancer often use their illness to help make people more aware of the risks of breast cancer as well as give others with the disease hope that they can overcome it as well.

Edie Falco

Edie Falco, star of both HBO shows "The Sopranos" and "OZ" was diagnosed with breast cancer when she was a young 40 years old, in 2003. She didn't tell people at first, but instead secretly went into treatment. She didn't reveal her cancer until after she was declared cancer free, but after that talked about openly. She was only secretive because she didn't want to be asked constantly how she was feeling and if she was all right.

Jaclyn Smith

Jaclyn Smith whom everyone knows as Kelly Garret, one of "Charlie's Angels" from the same-named TV show of the 1970s, was diagnosed with breast cancer when she was 56 years old in 2002. Most cases of breast cancer occur in women who are 55 years or older. Jaclyn was treated with a lumpectomy and radiation, because the disease was caught early during a routine self-exam. She works with groups like Susan G. Komen for the Cure and talks publicly about early detection and breast cancer risks.

Christina Applegate

Christina Applegate is one of the youngest and most vocal celebrities with breast cancer. She was diagnosed in 2008 when she was only 36 years old. The former star of "Married with Children" was filming her new successful hit show "Samantha Who?" when she was diagnosed. She tested positive for a gene mutation that greatly increases the risk of breast cancer. And because her mother had also been diagnosed with breast cancer more than once, making heredity just one more risk factor for the young woman, she opted to not just have the breast with the cancer removed, but to have both removed. This double mastectomy was a preventative measure to stave off any future cancer. Applegate speaks out for the available of MRI testing for women of all income levels, because it was this test that detected her cancer early and has probably saved her life by reducing her risk of the cancer spreading or recurring.

Melissa Etheridge

Rocker Melissa Etheridge is one of the most recognizable celebrities with breast cancer. She was diagnosed at age 43 in 2004. She had a lumpectomy, which removes only the lump, and then an aggressive course of chemotherapy to help prevent spread and recurrence. She caught the cancer while giving herself a self-exam in the shower, and gave a performance in 2005 at the Grammy awards while bald, after having lost her hair to the chemotherapy. Many people in her family died of cancer, so she is now an outspoken advocate of prevention and research.

Sheryl Crow

Rocker Sheryl Crow is also one of the most well known celebrities with cancer. She was diagnosed at 44 in 2006, and is outspoken about early detection. Surgery and radiation were her treatments, with no chemo because the cancer was caught in the very early stages.

Breast Cancer Realities in America in 2009

Seer Stats fact sheet estimates that 192,370 women will be diagnosed with breast cancer during 2009 and of those 40,170 women will die of cancer in the same year. The median age for women being diagnosed was 61 years of age, and none were under the age of twenty; 1.9% were aged 20 and 34; 10.5% were diagnosed between 35 and 44. From the ago of forty five the incidence rose sharply 22.5% were between 45 and 54; 23.7% were aged between 55 and 64. After 65 and between 74 the incidence dropped again to 19.6%, 16.2% were aged between 75 and 84; and finally 5.5% 85+ years of age. Just because their were only five and a half percent diagnosed at over 85 merely indicates that there were less women in that age group living.

The incidence was higher in white women as 123.8 per 100,000 women were diagnosed with breast cancer, blacks were diagnosed at 117.7 per 100,000 women, the Polynesians less still as 89.5 per 100,000 women , Hispanics had 88.3 per 100,000 women And native American Indian/Alaska Native had the least number affected. 74.4 per 100,000 women

From 2002-2006, the median age at death in America for cancer of the breast was 68 years of age, though 1.0% between 20 and 34; 6.2% between 35 and 44; 15.1% between 45 and 54; 20.3% between 55 and 64; 19.8% between 65 and 74; 22.8% between 75 and 84; and 14.9% 85+ years of age.

The age-adjusted death rate was 24.5 per 100,000 women per year. These rates are based on patients who died in 2002-2006 in the US.

In terms of trends the highest deaths occurred in the eighties when the mortality rate increased overall by four percent between 1980 and 1987, when the use of HRT in creased dramatically. Trend wise the greatest drop was between 1995 and 1998 when fears between HRT and breast cancer made a large number of women about the wisdom of HRT.

89.1% reached the magical 5-year relative survival rate between 1999-2005, which means that 10.9% did not make the five year breast cancer survival landmark. 90.3% of white women got to the five year breast cancer survival stage and only 77.9% of black women. However that is not necessarily an indication of genetics or ethnicity but could reflect the women who could afford the best treatment.

60% of breast cancer cases are diagnosed while the cancer is still confined to the primary site (localized stage); a third or 33% are diagnosed after the cancer has spread to regional lymph nodes or directly beyond the primary site; 5% are diagnosed after the cancer has already metastasized (distant stage) and for the remaining 2% the information was unknown. The corresponding 5-year relative survival rates were: 98.3% for localized; 83.5% for regional; 23.3% for distant; and 57.7% for unstaged.

If one estimates the 2004 -2006 figures it means that 1 in eight women will be diagnosed as having breast cancer at some point in their lives. On January 1, 2006, there were approximately 2,533,193 women alive who had a history of breast cancer. This figure comprises those that have active and those that were in remission.

In conclusion one in eight get cancer and two thirds of these are in the primary stages. Nearly ninety percent will celebrate the five year boundary. In general these figures are less bleak than they would first appear and there are definite grounds for optimism if you have been diagnosed with breast cancer this year.

Discover Current Trends in Curcumin and Breast Cancer Treatment

Research into curcumin and breast cancer treatment has thrown up some fascinating and potentially useful results. Many of the discoveries regarding curcumin and breast cancer treatment have relied on animal models and test tube studies. None has yet reached clinical trial stage although a preliminary trial has been done.

Curcumin (CC) has been found to inhibit the growth of cells of cancer of the breast grown in cell cultures in the laboratory. These include hormone-dependent, hormone-independent and multidrug-resistant cells

The compound also showed anticancer activity against mouse breast tumor cells.

In female rats, CC also showed a protective effect against chemically induced mammary cancer i.e. if the animals were given it prior, they developed less malignancies.

An early clinical trial has been reported where a topical application of a CC ointment was made in seven patients with cancer of the breast. The results showed that 71% of the patients had a positive response measured as a reduction in the size of lesions, less pain, less itching and reduced exudates.

One of the challenges that needs to be overcome if curcumin and breast cancer treatment is to progress to practical use is the low absorption: very little of the compound crosses the boundary from the digestive system to the blood stream.

One solution is the concurrent use of piperine which increases absorption by 2000%.

Another is the use of enteric coating which reputable manufacturers use to ensure that nutrient-containing supplements are delivered to the upper intestine rather than be destroyed by the acidic stomach enzymes.

Although we cannot declare that a cure has been found, these are promising results regarding the future of this treatment. One can therefore recommend taking CC more as a preventative rather than a cure.

Male Breast Cancer - An Under-recognised Disease

Male Breast Cancer is an under-recognised disease and is just as dangerous for men as it is for women.

It's rare in men but one thousand men are diagnosed yearly with having male breast cancer. Although more women are diagnosed yearly with it, the risk of a man dying with it is higher. This is because although the prognosis is the same, men tend to wait too long to seek medical help and so when diagnosed, the cancer is too advanced for successful treatment. The largest age group of men afflicted are between sixty and seventy years old.

Some Of The Male Breast Cancer Risk Factors Are:

  • Klinefelter's syndrome

Types Of Male Breast Cancer

Ducal Carcinoma in Situ or DCIS this afflicts one in ten cases this type of cancer in men. The cancer cells fill the ducts, but it doesn't invade through the walls of the duct into the fatty tissue of the breast or spread outside the breast. This type has a good curable rate with surgery.

Infiltrating or Invasive Ducal Carcinoma invades the fatty tissue of the breast through the wall of duct. Then spreads to other parts of the body. Invasive Ducal Carcinoma alone or mixed with Invasive or Situ breast cancer, afflicts 80% to 90% of male breast cancer victims.

Invasive Lobular Carcinoma or Lobular breast cancer is rare in men only afflicting about two percent. This is rare because men do not have a lobular tissue gland that produces milk in the woman's breast.

Paget Disease of the Nipple. This type starts in the breast ducts and spreads to the skin of the nipple. It made also spread to the dark area of the nipple known as the areola. The skin of the nipple usually looks crusted, scaly, and red, with areas of itching, oozing, burning, or bleeding. With the finger tips a lump may be found in the breast.

Paget disease mix with other types of breast cancer accounts for about one percent in women and a higher percentage in men. Since the man's breast is smaller than a woman's breast, this type of cancer in men starts closer to the nipple so spreading to the nipple is more likely. If no lump is found the biopsy shows Ducal Carcinoma but no Invasive Cancer the survival rate is very good.

Why Do More Men Die With Breast Cancer Than Women

Since it is rare in men, education, research, prevention, and yearly screening in men is not perform routinely like in women. Men lacking the knowledge about this type of cancer will wait too long before seeking medical advice. This delay is why more men die with it. When they seek medical help it is in its advanced stages which lowers the survival rate.

Treatment For Male Breast Cancer

Here again the smaller number of men with this type of cancer handicaps the medical profession. The lower number of men forces doctors to depend on research in women for the type of treatment to use for men.

Breast Cancer - How to Reduce Your Risk

This past month, in the United States the spotlight has been on "buying pink" to fund breast cancer research.

Even with all this funding, breast cancer in women in the United States has quadrupled in the last 40 years, causing physical and emotional pain for women and their families.

Dealing With the Fear of Breast Cancer

Every year when I go for my annual mammogram, I hold my breath. Will this be my year to be diagnosed?

I'm not just being paranoid. You see, my mom has been diagnosed with breast cancer three times. She's had more cancers than breasts! Both my grandmothers had breast cancer. So did a paternal aunt. And my younger sister had the disease. She died at the age of 47. So, my fear is justified.

From the moment I make the appointment for the yearly test, I am tempted to panic. Entering the diagnostic center is enough to make me sweat. Repeating my family history to the mammographer elevates my anxiety to the point of tears. I get undressed, put on the gown so that it opens in the front, go through the embarrassing motions of exposing first one breast, then the other to the clinician and the cold machine, get redressed, and wait the anxious days until I get the all clear signal--at least for one more year.

I know I am not alone in worrying about the diagnosis of breast cancer. One in eight women will have this illness sometime across the course of her lifetime unless a cure is found. Much is being done in the way of research, and there is reason to hope that a cure will come soon. So what should we do in the meantime about our fears and anxieties concerning being the next one who gets the dreaded diagnosis?

First, don't let fear stop you from being screened. Mammograms aren't fun, but because they can detect cancer in its very early stages, they do have the potential to save lives. Not knowing you have cancer is the greater danger. So, if you are over 40, talk to your doctor about getting mammograms. Have your physician perform manual exams. If you are in a high risk category, ask him or her about other screening methods like MRI, digital mammography, and ultrasound. Information is power, so get informed.

Second, take someone with you for moral support. I was asked to accompany a relative to her biopsy and follow- up appointment. I provided encouragement and support, but also was available to take notes about the doctor's advice. Sometimes, it is simply too much for the patient to take in all this information by herself and when she is feeling so vulnerable, so another set of ears can be very beneficial. A supportive hand to hold is powerful as well.

Third, know you family history well enough to make smart decisions, but don't dwell on it. Daily worry and anxiety causes stress, and that stress taxes the immune system and can set up an inflammatory response. This, in turn, sets the stage for cellular changes that can result in cancer. So, some researchers believe that worrying about getting cancer could become a self-fulfilling prophecy. Know what you need to know and do what you need to do to stay healthy, and leave it at that.

Fourth, become an advocate. Support breast cancer research through volunteering in your community or by donating time and money to national organizations trying to cure the disease. Not only can this lead to a cure in the future, but it can help you know that you are doing something proactive for yourself and for others right now. It keeps us from feeling so powerless against the disease.

Finally, see a therapist to sort out your feelings of fear, worry and anxiety. These common human emotions can become paralyzing if not given attention. It is very helpful to talk through your feelings, learn skills to calm anxiety, and find ways to distract yourself with the positive aspects of your life so that fear and worry don't set up camp in your mind, body and spirit. You are not your mother or your grandmother, or even your sister, so learning more about your identity, strengths, boundaries, and life can help you feel your own feelings about the disease and not take on the emotions of others as if they are your own.

Yes, breast cancer is a still a reality for too many women (and some men). There are no guarantees that you won't be the next one diagnosed, or that I won't be given the dreaded news at my next mammogram. But worrying and fretting about it won't change your health for the better. It will only set you up for future problems. Instead, channel all that energy into being proactive, positive, and purposeful with your life. If we all do that, a cure is sure to come!

Breast Cancer Stages

Breast cancer is divided into five stages. Stages 0-2 are considered "early", stage 3 considered "advanced", and stage 4 "late". Staging categories are important for predicting future prognosis, and determine optimal treatment recommendations.

Stage 0 is DCIS, or ductal carcinoma in situ. Breast cancer arises from the cells that line the milk ducts. When the cancerous cells are still contained inside the duct, it is diagnosed as DCIS. This can only be determined by a pathologist doctor looking at the tissue under a microscope. In general, when the DCIS lesion is small, there is no need to suspect cancer spread outside the breast.

Stage 1 is invasive or infiltrating cancer. Here, the cancer cells have broken through the duct wall and are found outside the ducts as well. In this case, doctors need to determine whether the cancer has spread to the lymph nodes. Stage 1 breast cancer must be equal or smaller than 2 cm in its invasive component, AND have no spread to lymph nodes. Often, the tissue removed at surgery contain DCIS in addition to the invasive cancer. However, only the dimensions of the invasive cancer count. If the patient needs to have multiple surgeries and the invasive cancer is found at more than one operation, usually the dimensions are added together to arrive at the final size.

Stage 2 has two subcategories. In stage 2A, the invasive cancer can be 2 cm or less and has spread to axillary (armpit) lymph node(s), i.e. positive node(s). Also, the invasive cancer can be as large as 5 cm, but has not spread to lymph nodes, i.e. negative nodes. In stage 2B, the invasive cancer is between 2cm and up to 5 cm and has spread to nodes. Here, cancer may measure even larger than 5 cm if it has not spread to nodes.

Stage 3 includes invasive cancer larger than 5 cm that has spread to lymph nodes. Also, cancer of any size that heavily involves the axillary lymph nodes to the point that these nodes are bulky and stuck together or stuck to other structures in the axilla (armpit) are in this stage. Tumor spread to lymph nodes either above or below the clavicle bone, or to nodes underneath the sternum (breast bone), also falls into this category. Furthermore, if the cancer of any size is attached to the chest wall (pectoralis muscle and/or ribs), it qualifies as stage 3. Inflammatory cancer, where the skin of the breast is red and swollen, is classified in this stage, regardless of size.

Stage 4 is invasive cancer found outside the breast and axillary lymph nodes, or "metastatic" to distant sites. At this stage, it does not matter how large the primary cancer in the breast is. Nor does it matter whether axillary/clavicle/breast bone lymph nodes have cancer or not. The most common sites for metastasis for breast cancer are bone and liver, followed by lungs and brain. Standard testing include bone scan and CT scan of the chest, abdomen and pelvis. More recently, PET scan is often done to look for cancer spread. Sometimes, a brain MRI or CT is also useful.

Understanding the Prognosis For Breast Cancer

Most types of cancer, including breast cancer, are grouped into one of four stages:Stage I The breast cancer is in one primary site and has not spread. Stage II The cancer has spread to nearby areas, but just around the primary site. Stage III The cancer has spread throughout the nearby area. Stage IV The cancer has spread, or metastasized, to other parts of the body, such as the liver, bones, or brain.
  • Further distinctions Within these stages, doctors make even finer distinctions of "A" and "B" -- so a tumor that is stage IIA is less advanced than one that is stage IIB. The distinction between cancer stages is often a very fine one, but it can be critical to making treatment decisions and knowing what to expect.
  • Reading the tests To determine a cancer's stage, doctors employ a series of tests. But once the test results are in, it becomes a matter of interpretation. This means doctors may disagree about the exact stage of this particular breast cancer and may even revise their opinions later as new evidence comes in. It's not uncommon for patients to be told their cancer is stage IV and fully metastasized, then have later tests reveal it to be stage IIIB with the metastases limited to one area of the body, meaning that it's more treatable.
  • Another kind of staging system For many types of breast cancer in which a tumor is present, the doctor may use an even more detailed staging system called the TNM (Tumor, Nodes, and Metastases) system, created more recently by the American Joint Committee on Cancer (AJCC). In this system, each of the three categories is assigned an individual staging number, so a T1N1M0 breast cancer means the tumor that is stage I, with lymph node involvement that is also stage I, and stage zero, or no, metastases.
  • Grading the cancer What can also be confusing is that, for some types of cancer, doctors use a grading system instead of or in addition to staging. Prostate cancer biopsy reports, for example, usually use what's called a Gleason scale to grade the malignancy of the tumor cells, with grade 1 being the least malignant and grade 5 the most malignant. Often they grade two different areas between 1 and 5 each, then add the two numbers together for a Gleason score that ranges from 2 to 10.
At any point in diagnosis and treatment, when the doctor is giving you this type of grading or staging information, don't hesitate to ask her to clarify what she's telling you. If the doctor says the cancer is stage III, for example, it's perfectly okay to ask exactly what that means for this particular type of breast cancer. You might also ask how the doctor arrived at her conclusions (which tests were run and what did the results show?), how this particular type of tumor tends to develop from one stage or grade to another, and what the cure rate is for this particular breast cancer at each stage. Another way to zero in on what the cancer prognosis information means is to ask the doctor if there are cancer cells in just one location -- such as a single tumor -- or if cancer cells have been detected elsewhere, and if so, where.

The type of cancer you're dealing with, as well as certain other factors (such as whether breast cancer tests positive for the HER2 protein, which tends to lead to more aggressive growth), can also be important in understanding the prognosis and treatment. Certain cancers, even rare ones, may have a specific type of treatment available that offers a more optimistic prognosis. It was big news a few years back, for example, when the tumor-suppressing drug Gleevac was found to work very well against certain rare gastrointestinal tumors and the even rarer chronic myeloid leukemia (CML) by blocking specific enzymes that fuel cancer growth.The other thing to keep in mind is that cancer prognosis information and treatment options are inextricably intertwined. It might seem like extremely bad news when the doctor tells you that your mother's breast cancer is HER2-positive, which studies have shown carries a worse prognosis than HER2-negative cancers. On the other hand, newly available drugs such as trastuzumab and lapatinib can be very successful against HER2-positive cancers and have no effect on HER2-negative cancers -- so finding out that the cancer is HER2-positive opens the door to additional treatment options.

The other wild card is that you can't know in advance how well you or the person you're caring for will respond to a particular treatment. One of the things that makes cancer so mysterious and frustrating is that doctors can't predict how effective a particular treatment will be because patient response is so individual. For each diagnosis and treatment option, the patient population responds along a bell curve, with the larger group of patients in the middle of the curve responding "typically" and a small group on either end who respond either much more positively or less well. Many breast cancer patients like to set a goal for themselves of "beating the bell curve," because no matter what the general prognosis for the majority of patients with a particular diagnosis, some are going to fall on the side that beats the odds.

You or the person you're caring for may well turn out to be one of the patients who is quite responsive to breast cancer treatment, but unfortunately the only way to find out is to give it time. If a treatment doesn't work, it's still not reason to despair -- it's likely the doctor will have another approach to suggest.Some doctors tell patients to think of their cancer treatment like a bag of tricks. For most types of breast cancer, doctors have a number of different treatment options they can try. If one doesn't work, they'll switch to another, and that just might be the one that does it. It can be hard to have patience and wait for positive results, but fully understanding the cancer prognosis will help you know how much room there is for hope.
